Program funded by Connecticut State Department of Mental Health and Addiction Services provides case management for chronic relapse substance abusers. Participants are usually adults who frequently use acute services such as detoxification, and who are known to the treatment providers as chronic relapsers. Case managers provide linkage to treatment services, assistance with problem solving, advocacy for entitlements and housing, and other supports designed to improve quality of life in order to reduce incidences of relapse.
Ages 18+; Two or more admissions to detox or inpatient rehabilitation within the last 12 months or frequent use of community resources (i.e. ER, police, shelter); Clients currently receiving SAGA are not eligible; Meet the DSM IV criteria for substance dependence; At least 3 of the following: 1. History of needing continuous supportive treatment; 2. Current housing or homelessness increases risk of relapse; 3. Lack of financial resources; 4. Other condition(s) increases risk of relapse; 5. Involved in or appropriate for specialized services, i.e. DCF, AIC, probation.
