New Medicaid Applications for Long-term nursing home services and support (LTSS), https://portal.ct.gov/DSS/Health-And-Home-Care/Long-Term-Care/Long-Term-Services-and-Supports---LTSS/Apply, for Connecticut residents, can be mailed to three processing centers that the Department of Social Services has set up at the DSS field offices listed below in Waterbury, Bridgeport, and New Haven. NOTE: These Centers receive and process new Mediaid applications from specific cities and towns for all nursing home care facilities.
**The Greater Hartford LTSS Application Center will process all new Medicaid applications for home- and community-based services statewide (Medicaid waiver programs). See list of cities/towns below. - Application packets with as much documentation as possible should be mailed directly to the appropriate LTSS Application Center. These Centers will process new applications only.
**Long-term services and supports redeterminations and interim changes, will continue to be processed through the statewide ConneCT mode. All paperwork for redeterminations and interim changes, should be sent to the DSS ConneCT Scanning Center at P.O. Box 1320, Manchester CT 06045 (not to the new LTSS Application Centers). - The LTSS Application Centers will also support admissions and discharges affecting area nursing homes in the ASCEND system. For questions specific to a pending case after an application is made, clients should call the assigned caseworker directly. Applicants will be provided a worker’s contact information once the case is assigned. For all other general calls or questions, the DSS Benefits Center staff will serve you at 1-855-6-CONNECT (1-855-626-6632).
AN APPLICANT MUST: be a citizen or a qualified non-citizen of the United States, be a resident of Connecticut, provide a Social Security number, cooperate in establishing eligibility, assign a claim to any possible source of income or resource, not have transferred assets in order to qualify for assistance

Cash or medical insurance programs include Temporary Family Assistance (TFA); State Administered General Cash Assistance (SAGA); State Supplement for the Aged, Blind and Disabled (State Supplement); Medicaid; and other medical expense or personal care assistance programs for special populations. DSS also administers funding for child support enforcement services, energy assistance, rental assistance, emergency housing programs, burial funds for the indigent, and protective services for the elderly.
