CHOICES (Connecticut’s program for Health insurance assistance, Outreach, Information and referral, Counseling, and Eligibility Screening) helps Connecticut’s older adults and persons with disabilities understand their Medicare coverage and healthcare options. Certified CHOICES counselors across the State provide free, objective, person-centered assistance so Medicare beneficiaries can make informed choices about their Medicare options and related benefits. CHOICES provides counseling on all types of Medicare and related insurances, including Medicare Part A, Part B, Part D, and Medicare Advantage. CHOICES counselors can help Medicare beneficiaries understand their Medicare Part D (prescription drug) options and can help them select the Prescription Drug Plan that best suits their individual situation.
Types of assistance includes counseling and information about Medicare and related insurance programs, outreach, information and referral to services, counseling about resource options, eligibility screening, and application assistance for state and federal benefits and/or support programs, including the Medicare Savings Program, Medicaid, and Extra Help/Low-Income Subsidy programs. CHOICES also provides community outreach, application assistance, and information and referral services on a variety of topics including long term care services and supports.
CHOICES is managed by the CT Aging and Disability Services State Unit on Aging through a partnership with Connecticut’s Area Agencies on Aging and the Center for Medicare Advocacy, with funding from the State of Connecticut and the Administration for Community Living.
