Program provides homeless individuals, including families, with outreach, engagement, screening, mental health and substance abuse assessments and intakes. Other services include clinical/cases management; assistance in obtaining and coordinating social services, entitlements and income support services; housing assistance; medical services; referrals to community mental health and substance abuse programs and other services as needed.

Off-site service providers include Bridges Extended Day Program, The Center for Cancer Care, The Center for Youth and Families, Charlotte Hungerford Diabetes Center, Charlotte Hungerford Pulmonary Rehabilitation, Foster Care Clinic of Northwestern Connecticut, Hungerford Emergency and Medical Care (Winsted), Hungerford Regional Sleep Laboratory, and CHH Medical Walk In.
