SOCIAL SECURITY: TO GET A NUMBER/TO CHANGE YOUR NAME/TO REPLACE A CARD

HOW TO GET A SOCIAL SECURITY NUMBER: Social Security Numbers are issued through the Social Security Administration. An application can be obtained from a local Social Security office or by downloading Form SS-5 (www.socialsecurity.gov/online/ss-5.pdf) from Social Security’s website. If the applicant is 18 or older and has never had a Social Security number, he or she must apply to a Social Security office in person. Along with completing an application, documents need to be provided that state the applicant’s age, identity, and citizenship or lawful alien status.

HOW TO CHANGE A NAME ON YOUR SOCIAL SECURITY CARD: To change your name on your Social Security card, you must also complete Form SS-5. You need to provide recently issued documents that show your legally changed name such as a marriage document, divorce decree, court order for a name change or a certificate of naturalization. The new Social Security card will show your new name, but will have the same number as your old card.

HOW TO REPLACE A LOST SOCIAL SECURITY CARD: Form SS-5 must also be completed to replace a lost or stolen Social Security card. At least one recently issued identifying document, such as a driver’s license, employer ID card, passport or school ID card, must be shown as proof of identity.
