SHELTER PLUS CARE
PROGRAM ADMINISTRATION: Conn. Department of Mental Health and Addiction Services (DMHAS)
PROGRAM DESCRIPTION: Shelter Plus Care is a Conn. Department of Mental Health and Addiction Services (DMHAS) program that provides subsidized rental housing and psychiatric/health case management for persons who are homeless and who have a history of psychiatric disability, substance abuse, dual diagnosis, or who are HIV positive. DMHAS subcontracts with community agencies to link eligible people with Shelter Plus Care. Not all towns in Connecticut are covered by Shelter Plus Care.
Community providers that administer Shelter Plus Care funds are as follows:
- REGION 1: Greater Bridgeport Community Mental Health Center, F.S.DuBois Center (Stamford)
- REGION 2: Conn. Mental Health Center (New Haven), River Valley Mobile Services (Middletown), Rushford Center (Meriden)
- REGION 3: United Services (Willimantic)
- REGION 4: Capitol Region Mental Health (Hartford)
- REGION 5: Danbury Mental Health Authority
WHO IS ELIGIBLE: Homeless adults who are HIV positive, or who have a history of psychiatric disability, substance abuse, or a dual diagnosis.
