ACQUIRED BRAIN INJURY (ABI) WAIVER
TRAUMATIC BRAIN INJURY WAIVER

PROGRAM DESCRIPTION

The Connecticut Department of Social Services administers a special Medicaid waiver program for income eligible adults who have an acquired brain injury and who, without waiver services, would have to live in a long term care facility.  The program pays for services such as case management, personal care assistance, chore services, respite care, vocational supports, housing supports, and other services that are necessary to keep the individual living in the community.

ELIGIBILITY

• Adults ages 18-64 who have acquired brain injuries
• Monthly income under 300% of the maximum monthly SSI benefit, liquid assets of $1,600 or less
• Must meet "Level of Care Requirement" which means that without the waiver services the individual would have to be placed in a long term care facility
• Must be able to participate in the development of a Service Plan that offers a community alternative to institutional living

TO APPLY:
Call Social Work Services Team at the DSS Central Office or your local DSS regional office/suboffice to ask for a Waiver Request Form.

NOTE:
This is not an entitlement program; once DSS has reached its participant quota, names are placed on a waiting list.
